Eastcare Hawthorn Primary Care Project
The Hawthorn Primary Care Project assists in service access and provision for young people and adults living within the City of Boroondara in boarding and rooming houses, supported residential services, bedsits or similar housing and to those who are homeless. The Project employs case outreach support workers with backgrounds in housing, mental health, alcohol & drug services, aged care and social work to provide outreach support and crisis intervention as well as to identify client needs and develop strategies to meet those needs. A Salvation Army chaplain and a Homeless Persons' Program Community Health Nurse are available. Appointments is required.

Hi I need some help for basic needs as I have nothing until I start and paid with new part time job - which I presume will be within 2-4 weeks. My housemates are helping with rent but that is all help that I've got. I've also lost contact for help from families and friends. My evidence documents were not enough to receive support from Vinnies and other organisation.

Hi my name is Emma and I am a single mum of two teenagers and I am in need of help with a food Voucher and fuel voucher. I am and have been struggling financially for quite some time now and don't like asking for help, with minimum help from centrelink due to the kids ages and not being classed as a single mum and only one shift a week at work my situation has become worse and am not breaking even to pay rent and my other bills. There is no money left over for groceries or fuel. Can someone help please?
