Aged Care Assessment Team determines the level of care a person requires, provides information on suitable options and assistance with arranging residential care or In Home community services.This service provides a comprehensive holistic health assessment of older people, dementia assessments, carer support advice on a range of services and care plans in conjunction with your local doctor, and other service providers where appropriate. nDay Care Centre for Low-Mod level Frail Aged or Dementia (can take wanderers premise is secure) <p> Day Care operates Tue-Fri 9.30am-3pm. Activities include craft, gentle exercise, games etc. People visiting area can utilise day care respite for short term care. Centre also runs a fall prevention program, has gym area for visiting physiotherapist to work on mobility and gait improvements.
