About Albany Outreach Support

Provide support to people in their own home who suffer from a mental illness. Have one unit for limited stay (two bedroom house- this is a step-down facility) up to 3 months.nClients are referred through their case managers or through GSMHS.nSupport activities include: independent living and household management skills; accessing community services to foster development of social skills, confidence and self-esteem.nThis service mainly provides outreach to people with a mental illness in their own homes, or other community residential/facility settings. In-home support is individualised around the client's own goals and lifestyle with an emphasis in learning and or maintaining skills with a view to recovery. They also offer respite in-home for the carer to have a break.They also have a community sports and recreational program for their clients which is also an out of home respite.
