On average the Team travels a 200km radius from York . Services provided are:- Support and Case Management- The Team will continue to offer support to the family throughout the time the person with dementia is living at home. Ongoing respite program - Regular community-based respite is offered to families through a variety of models including: In-Home Respite Host Family Respite This provides a social club for an average of three people in the earlier stages of dementia in the home of a care worker. Each home is known as a "Host Club" and clients usually enjoy various activities such as art and crafts.

Changes made to the scheme in September 2003 help to protect the interests of legitimate users and prevent the fraudulent use of MPS cards. All applications for new or renewal of MPS cards must be signed by a medical doctor. Doctors are aware that the fraudulent use of MPS cards disadvantage the people the scheme is designed to help, and will not sign the medical certificate unless you genuinely meet the requirements.
