The Hawthorn Primary Care Project assists in service access and provision for young people and adults living within the City of Boroondara in boarding and rooming houses, supported residential services, bedsits or similar housing and to those who are homeless. Community based support group which aims to provide information and support to sufferers of neurological disorders (including Motor Neurone Disease, Parkinsons, Tourette Syndrome, Multiple Sclerosis, Epilepsy, Huntingtons Disease, Cerebral Palsy, Dementia/Alzheimers, Rhett Syndrome, Acquired Brain Injury) and their families, friends and carers. Also provides Case Management support from diagnoses onwards. Support Group meets every 2nd Wednesday of the month, 1pm at the Musicians Club. (Current at 31/03/08)
