The York MDRT was created to provide education, support and respite to carers and people with dementia. The service, which is funded by the Commonwealth National Respite for Carers, assists families in accessing community-based respite services based in the Central Wheat belt and part of the Upper Great Southern Region. On average the Team travels a 200km radius from York . Services provided are:-nSupport and Case Management- nThe Team will continue to offer support to the family throughout the time the person with dementia is living at home.nOngoing respite program - Regular community-based respite is offered to families through a variety of models including:nIn-Home RespitenHost Family RespitenThis provides a social club for an average of three people in the earlier stages of dementia in the home of a care worker. Each home is known as a "Host Club" and clients usually enjoy various activities such as art and crafts.
