About Cockburn Community Care - CACP

The Community Aged Care Packages are planned and coordinated packages of care to assist people to remain living in their own home. The packages are specifically designed to cater to the individual and are flexible to meet varying needs eg:nPersonal Care nMeal Preparation nHome Help/Cleaning nTransport Services etc nTo be eligible to receive a Care Package, the client must be assessed by ACAT. A referral for this can be obtained from your doctor. These services are subject to fees. Unlike other programs within Cockburn Community Care, this service does have an after hours call out facility for emergencies only. Please contact the main office number and ask to speak to Tracey Ramshaw (0419 836 796) - Service CoordinatornCACPs available only to City of Cockburn.There are 35 CACPs(Gen, ASTI and CALD).The languages of the CALD are Italian, Portuguese and some Aboriginal languages.
