About Eastcare Hawthorn Primary Care Project

The Hawthorn Primary Care Project assists in service access and provision for young people and adults living within the City of Boroondara in boarding and rooming houses, supported residential services, bedsits or similar housing and to those who are homeless. The Project employs case outreach support workers with backgrounds in housing, mental health, alcohol & drug services, aged care and social work to provide outreach support and crisis intervention as well as to identify client needs and develop strategies to meet those needs.nA Salvation Army chaplain and a Homeless Persons' Program Community Health Nurse are available.nAppointments is required.
