Home and Community Care offers the following services :-nDay Centre activities & outings nCarer support and respite nTransport nRecreation & social activities nHome Help nHousekeeping and laundry nInformation nHandyman/ gardener nAssessment nHome Visits nShopping nMeals on Wheels nThe adult day care activities run every day for each target group. Esperance Home Care also offers a community nursing program, which is a maintenance role for clients with long term health issues who may need monitoring, this includes the hospital to home service - visiting clients or potential clients (post referral) in Hospital to determine what services may be needed to assist clients on discharge, clients are visited at home post discharge twice and may have more regular community nursing visits is required in the long term.nCommunity nursing also has an ongoing support/information sharing role with clients and their Carers living with dementia, memory loss and Alzheimer's.
