About Fremantle Aged Care Assessment Team (ACAT)
The ACAT Team is a part of the Dept of Community and Geriatric Medicine.nThe Aged Care Assessment Team helps older people and their carers work out what kind of care will best meet their needs when they are no longer able to manage at home without assistance. ACATs provide information on suitable care options and can help arrange access or referral to appropriate residential or community care.nEligibility for an ACAT AssessmentnPatients who are eligible for an ACAT assessment need to:-nBe Aged! 70 years or more or 50 years or older ATSI (there are occasional exceptions) nBe Medically Stable!!! Those patients who are receiving acute care or engaged in a rehabilitation program are not eligible . nHave significant physical, medical, social or psychological care needs that cannot be met more appropriately through other services nBe a voluntary patient. Not be eligible for assistance through Disability Services Commission. Give consent for an ACAT assessment.
