This service provides specialised medical and clinical services and support for older people, Frail Aged and people with an aged related physical or mental disability. nThe GSAHS - Aged Care Service offers the following services:n- Aged Care Assessment Team - provides assessment for admission to Commonwealth funded Residential Care Facilities,Community Aged Care Packages and Extended Aged Care Packages. Geriatrician and visiting psychogeriatrician and Dementia Specialist Services.n- Geriatric Medical (Specialist Consultancy),n- Day Activity/ therapy centre.n- Multidisciplinary aged care ACAT support Team includes- Occupational Therapist, Social Worker, Speech Therapist and Physiotherapist.n- Active Ageing Program, Falls Clinic and Transitional Care Program
