The community Nursing Service offers a wide range of clinical nursing services to eligible people who have chronic conditions, who have recently been discharged from hospital and /or who have continuing support needs. Service works with a range of health and community services to promote, maintain or restore the health and well being of the individual. They also encourage individuals and the community to take responsiblity for their own health and to engage in activities to improve their health and well being.nAlso accomodates an Aged Care Assessment Team & Child & Family Health Service nurse.nThe Community nurses cover the old Holbrook Shire boundaries while the ACAT/Child & Family Health Service covers further afield.
