Please contact the service Mon - Fri 8.30am- 5pm. Registered clients are provided with an out of hours contact number. Palliative Care is a community based service. Interdisciplinary team provides medical, nursing, social work and volunteer co-ordination to all eligible clients. Palliative care focuses on quality of life. Early intervention and referrals are encouraged.
