HACC and CACP services are offerred by Silver Chain at their Toodyay and Gingin/Bindoon base. To access information or make a referral call the Silver Chain Customer Centre on 1300 650 803 (local call fees apply), or ask to be put through to your local service centre. Provides home nursing and home help services, Palliative and personal care, diabetes education , home maintenance, social support , meals delivered centre-based day care and in home respite . Weekend emergency service available. Covers Shires of Gingin, Chittering, Bullsbrook and surrounds, Bolgart and Muchea.nProvides x31 CACPs(Gen) to the Shires of Chittering , Gingin, Bullsbrook and surrounds, Bolgart and Muchea. x10 CACPs available to Toodyay and x10 to Gingin. Palliative care services only at the Gingin/Bindoon base.
