To access Silver Chain's wide range of services you can make a referral by calling the Customer Centre on 9242 0347. The Silver Chain Customer Centre lines are open 24 hours a day. Anyone can make a referral to Silver Chain or be referred by a family member, carer, GP, hospital or other health service.
