About South West Gippsland Post Acute Care Service

Post Acute Care describes the management and provision of short term planned care for patients and/or carers with more complex needs during their recovery from an episode of acute illness or injury. Short-term care can be provided within the first four (4) weeks of discharge. Provides individually tailored packages of health and community care services for children, young people, adults, families and the elderly in order to provide additional support when needed and assist recuperation following discharge from an acute public hospital or any publicly funded emergency department or rehabilitation facility. nServices include home nursing, personal care, home care and some allied health.
