The Wagin Home and Community Care programme is jointly funded by the State and C/wealth Govts and is administered by the Shire of Wagin. HACC covers the Shires of Wagin and West Arthur.The target groups for home support services are for the frail aged and younger people with disabilities and their carers. Must be HACC eligible to receive services. The services offerred are :-nAssessment ; Centre Based Day Care ; client Care Coordination ; Domestic assistance ; Home Maintenace ; Nursing Care (Home) ; Personal Care ; Respite Care ; Social Support ; and Transport. There are X4 CACPs(Gen, ATSI,CALD) that cover the areas of Wagin and Arthur River. HACC covers the Shires of Wagin and West Arthur. Services Provided by nWagin Community Health n Child Health Services n Community Health n Health Promotion and Education n Home and Community Care (HACC) n Immunisation nAlso Meals on Wheels or Wagin MOW.
