Community Options provides individual case management for people in the HACC target group who are at risk of inappropriate or premature placement into residential care. Client's require a package of services, they have complex care needs and high need for ongoing monitoring and co-ordination of services. Often client's require a safety net service as the complexity and fluctuating nature of their care needs contribute to their difficulties in gaining access to the full range of necessary services.
