About Adult Mental Health Service - Acute Home Based Treatment Team - Northern Sydney Central Coast Health

This team provides care for people experiencing acute symptoms of mental illness. Care is provided in the clients home or through community health centres. The team follows up all people who have been discharged from the Inpatient Units. The length of care with this team will depend on the clients individual needs. Care can be provided for up to 8 weeks, after which people may be referred to one of the other Adult Mental Health Teams to continue their recovery.
