This service provides a comprehensive holistic health assessment of older people, dementia assessments, carer support advice on a range of services and care plans in conjunction with your local doctor, and other service providers where appropriate. There is a waiting list for this service but the service is accepting referrals and there are no special requirements for referral to this service.
