Assessment of aged in relation to service needed to remain independently in their home if that is their wish. Referrals made to appropriate services. Determination made regarding level of care needed and ACCR completion to allow clients access to CACP's and flexible care (EACH, MPS or Trasitional care) and residential respite / permanency. Also low>high assessments in relation to ACF. I have electronically sent information brochure attached in this e-mail. Availability waiting list is approximately 3-6 weeks. Servicing Muswellbrook, Upper Hunter and Singleton LGA including Willow Tree. Registered nurses, OT, social worker, geriatrician on staff.
