Provides a comprehensive rehabilitation program designed to be undertaken by recently hospitalized patients in their own home. Coordination is provided by a case manager who is assisted by the therapists required for treatment needs. It is available to people living within 20 kms of Ballarat and is for people who require rehabilitation after hospital admittance.
