Community Aged Care Packages (CACPs) aim to assist older Australians with low care needs, to remain living in their own homes. The type of support provided can include: meal preparation, personal care, house cleaning, social interaction, transport to appointments, etc. The service includes case management and is about meeting the individual needs of clients to assist them with their independence and health and wellbeing.
