MONITORING: In consultation with client we develop on-going Care Planning, Referrals and Reviews. This could involve - Monitoring health and well-being, nutition, medication and other idenified monitoring requirments.nSOCIAL SUPPORT: Can include the planning of daily schedules and coordinating appointments as the need is identified, outings, social visiting. Priority is given to persons with memory loss who: * Live by themselves * Who have no Social Supports * Whose Carer has gone into crisis.
