124 CAPS, including dedicated packages for muslim and chinese/vietnamese, 43 EACH packages including 12 IT EACH monitoring of chronic illness with IT (Telemed Care). 12 Dementia EACH. Support Veterans Home Care with respite and personal care and domestic assistance. Transitional Care has 15 residental and 20 community places: step down program for people who are coming out of hospital following an acute episode and require assistance and rehabilitation. CALD Overnight Respite O/N and day respite in the persons home. Sundowners program respite for carers during the evenings and Saturdays. Host day Care respite for carers caring for people with dementia. The person can access respite in a Host own home during the day targets working carers.nDaybreak Program providing comprehensive support for working carers-includes assistance with transport for appointments, dietician and podiatrist.
