About BCS - Transitional Aged Care Program

Transitional Aged Care Program (TACP) is a collaboration between Greater Western Area Health Service (GWAHS) and Baptist Community Services (BCS).nThe service provides up to 12 weeks support for older people who have been in hospital.nWhile on the program, clients receive a mix of the following: Case management, therapy, care input and medical advice.nTACP is aimed at older people who are at the conclusion of a hospital stay and require more time and support in a non-hospital environment to complete their restorative process.
