Program provides short term, Government orientated HACC Services to HACC eligible clients were the client cannot get the services required from the existing HACC network. Approval of referral is by case discussion by Clinical Committee. Service hours detailed on service request and are reviewd regularly . Brokerage Model.Activities: Services delivered in the home. Weekend Service can be negotiated if needed.Special Conditions: : Short term needs in areas of Domesitc, Nursing or Personal Care. Live in district covered; attainable discharge goal; not receiving other funding besides HACC e.g. CCP, DVA etcAdditional Fees: As erquested by Service Provider only.Referral required: From health professional who has assessed client for eligibility.Other : Client needs to be aware of short term nature of funding
