Blue Care services are targeted at people who are frail aged or younger disabled. Other services include Pastoral care and counselling, clinic facilities at West End, staff development and education, palliative care, wound management, diabetes management, continence service, volunteer services, Veterans Home Care, Community Aged Care Packages, respite for carers funding, personal care and social support services to people who live in boarding houses and supported accommadation. All services based on assessed need & agreed care plan. Majority of services delivered between 7am-4pm. Essential nursing care may be provided on week-ends & evenings.nHACC Eligibility applies - if not HACC eligible may explore other funding source or pay fee for service. Fee schedule applies for private [non funded] clients. Referral via Central Intake Unit 3722 1740 - Doctor's referral required for medical treatment
