Chronic and Complex Care (C and CC) services are aimed at streamlining and improving care of patients with chronic heart failure and better use of resources, reducing unplanned hospitalisations and improving quality of life.nService for people with diagnosis of heart failure, ventricular failure, cardiomyopathy.nAction plan for treatment and ongoing care - prevention strategies.nProgram at Bulli Hospital of group exercises, discussion, education, counselling.nA homebased service of education and prevention strategies - aim to improve life quality.nSupport for aged care facilities also provided. nPatients can refer to the service by contacting the service coordinator, who will notify the patient's GP, cardiologist and/ or physician of their patient's enrolment.
