About Canterbury Aged Care Assessment Team (ACAT)
This multidisciplinary team includes Social workers, Registered Nurses, Physiotherapist, Occupational Therapists, Geriatricians, Continence Advisor and a psycho geriatric team. We provide Comprehensive assessment, referral to community services, information and support to older people and their carers. We also conduct assessment for entry into an Aged Care Residential facility, i.e. Hostel or Nursing Home, assessment for Community Aged care packages (CACP'S) and Extended Aged Care packages (EACH). The team has a continence advisor, who can provide information on the management of bladder/bowel problems.
