Provides case management and co-ordinated service approach using brokerage to link people with services to enable them to remain living independently in the community. For example: home nursing, respite care, home maintenance and repair, home help, personal care and meal services. Provide in home HACC assessment. Aims to avoid premature or inappropriate institutionalisation. Cannot access if already receiving other support services
