About Care Assessment Consultants - HACC Home-Based Independence Program (HHIP)

A 1-12 week individual program to improve the overall health, independance and social interaction of people with chronic health conditions aged 60-years and over. A multi-disciplinary team of health professionals assist by:n1. Providing a comprehensive health assessment in your home.n2. Providing assistance in setting your goals to achieve independence.n3. Establish a plan of care and arrange services which will improve your ability to carry out daily activities.n4. An individualised exercise program which will improve your strength, balance and reduce your risk of falls.n5. Education about managing your health needs on a daily basis.n6. Information to improve your management of medication.nHACC fees apply. Not suitable for people with acute conditions: the Post Acute Package program is tailored for people with these needs.
