About Care Connect - ComPacks Eastern Sydney & St.George

ComPack is a case managed package of care arranged prior to discharge from hospital. Care period lasts up to 6 weeks. It involves a community assessment and case management of targeted people being discharged from hospital with the purpose of rapidly assembling individualised community care packages designed to meet each persons assessed clinical and support needs. Support available includes: domestic assistance, personal care, meals, social support, centre based day care, transport, respite and other services as required. This ComPack is for residents of City of Sydney, Botany, Eastern Suburbs & St.George who need care arranged prior to discharge from most public hospitals.
