About Care Connect - Dementia Options Program

Dementia Options provides a case management and brokerage service for people with dementia who live alone. This includes those with long standing disabilities who also have dementia. The program provides care planning, monitoring and coordination of brokered services to enhance independence and prevent premature admission to residential care.
