The Independence At Home Program provides case management and brokerage services for adults who have disability and who would benefit from independent living training to increase their independence in the community. The program provides a range of services including: brokerage, care planning, coordination and monitoring to enable clients to enhance their independence and living skills. Fees are negotiable and nominal.
