About Care Connect - Sydney Indigenous Program (SIP)

The Sydney Indigenous Program is for Aboriginal Australians or Torres Strait Islander Australians who are over 45 OR younger and have a disability and would benefit from some social support. Social support may include help with:nWriting personal letters and resumes, filling out centrelink and financial forms.nShopping - taking you to the shop for groceries or clothes, helping you shop for them. Taking you to the chemist and to pick up medications.nBanking - using an ATM or bank transactions, helping you fill out forms.nTransport to medical appointments with your doctor.nFriendly visiting- having a coffee or a chat, going for a walk, having lunch, seeing a movie or attending groups.nYou will have a case manager whom will work with you to sort out any help and support you need as well as facilitate changes along the way.
