About Care Connect Transitions Options Program

The Transition Options Program provides case management support for people with long term disabilities who are ageing, and their carers. It aims to facilitate the transition from employment and/or Day Programs to a more age appropriate option with a focus upon community integration. The program aims to maintain the person's skills and independence through coordinated and individually focused support, together with brokered services.
