Offers multidisciplinary assessment and interventions including individual as well as group therapy and education and support groups, home safety assessments and exercise program. For aged people who have difficulties in at least 2 areas, eg mobility, activities of daily living -showering, dressing, cooking, communication, social isolation, etc. Gentle exercise classes, activities group, leisure group. Occupational therapist,nphysiotherapist, speech pathologist.
