Transition care consists of case managed packages of care. n*ComPack (Community Case Management Packages): provide short term case management and brokerage services for people with complex care needs discharged from designated hospitals such as Royal North Shore Hospital and Ryde Hospital. Case management supports the person in their home for up to 6 weeks. n*SAFTE (Sub Acute Fast Track Elderly Care): supports the case of older people at home whose medical condition is starting to worsen. The package provides case management and support for up to 6 weeks. n*Transpac: provides case management and services for people who are assessed as needing residential level support when leaving a public hospital and are over 70 years.
