About Central Coast Aged Care Assessment Team (ACAT)

The Aged Care Assessment Team is a team of health professionals who provide expert assessment and advice regarding the long term care needs of the frail aged . The main professional groups represented are occupational therapists, nurses and social workers. ACATs assess the whole care needs of an individual using a multi disciplinary and multi-dimensional approach. As part of the assessment process a person's medical, physical, social and psychological needs are considered before care approval is made. The needs and wishes of the person being assessed as well as those of his/her carer and family are an important part of an ACAT recommendation. Clients need to be assessed by ACAT before they can receive a Federal Government subsidy for Residential Care (either low or high care). A Community Aged Care Package (a coordinated package of community care, equivalent to low level residential care) and Extended Aged Care in the Home (the equivalent of high level care in the client's home)
