Provides a community based paediatric therapy service to children and families 0-18yrs with a disability or medical condition of a physical nature, learning difficulties and developmental delay and who live at home. Identify, co-ordinate and monitor service/supports that children with a physical disability and their families and carers may require to participate in the community. Physiotherapy, occupational therapy, speech and language pathology, social worker support.
