About Community Aged Care Packages - Homeless

Support to help older people who are homeless or at risk of becoming homeless with day-to-day living. To find out exact eligibility for which support services are required, an assessment will be conducted by the CACP Coordinator. Following the assessment, a Care Plan will be designed and agreed upon for services provided. The Care Plan is reviewed regularly to make sure that clients are getting the help needed. Services may include; hot showers; laundry; pastoral care; meals; shopping; housework; social activities; transport and help getting to and from appointments.nThere is a waiting list.
