Transitional Care Program being established; central intake service, adult, home care, medical aids, oxygen, continence, spectacle supply scheme, advocacy, nutritionists, social workers, physiotherapist, occupational therapists, nurses, indigenous health worker, paediatric health program, alcohol and drug program, cardiovascular, cancer, diabetes, asthma, injury prevention, aged, disabilities. (child health, mental health and ATODS are all in the same building, have seperate lines but can also be contacted through this number.)
