About Community Health - Social Work

Provide a Social Work service to individuals, families, groups and communities, with a focus on developing innovative services.nSocial Workers in Community Health:n . assess and analyse social and health needs of diverse communities and implement and evaluate programs to meet needs;n . initiate and contribute to the development of innovative health policy;n . provide a casework service to individuals with complex needs, both short and long term, drawing on a range of therapeutic strategies;n . initiate, plan and co-ordinate group work, community development and health promotion; andn . act in a consultative and educative role to community and health workers and organisations on the social aspects of health.
