Community Options Projects have the primary purpose of ensuring that the most vulnerable and dependent members of the community, whose needs cannot be adequately addressed through the existing service system and who require case management, are provided with appropriate, flexible and co-ordinated support and assistance, through a comprehensive case management approach.nServices may include:n . home nursingn . home helpn . home maintenancen . personal caren . respite for carersn . transport (for which HACC fees may apply).
