About Compacks - Albury & District

ComPacks is a case managed package of care provided for up to 6 weeks after discharge from hospital. The length of service depends upon the amount of support required. This service is for people who need two or more services in order for them to return home safely with appropriate care in place. Services are flexible and may include;Case Management, Domestic Assistance, Personal Care, Meals, Social Support, Transport, Respite Care, Small items of equipment and Home Modification and Maintenance. Where ongoing assistance is needed after the six week period, clients will be referred to other community services.
