Chronic and Complex Care - chronic disease management, complex care, mens shed and Planned Activity Groups.n3. Primary Care - counselling and casework, dietetics and nutrition, medical practice, occupational therapy, physiotherapy, podiatry and refugee health.n4. General dental.n5. Health Promotion - community development and community liaison.
