About Dept of Ageing, Disability & Home Care - Community Participation/Transition to Work

The Community Participation project provides pathways for young school leavers Yrs 10,11,or 12, with moderate to severe disabilities to access education and employment opportunities with support to meet their individual needs. nFocuses on creating links to jobs, employment assistance programs and vocational training, and aims to address some of the barriers that prevent people with disability experience in these areas. School leavers are assessed along with the school and if approved receive a package to purchase services. The approved service providers develop individual programs that must promote community integration, self esteem, employment and independence.
