About Diabetes Direct - Northern Alliance HARP CDM Program

Diabetes Direct is a single point of contact for GPs in the Darebin and Whittlesea areas to co-ordinate the assessment, treatment and management of their diabetic patients.nIt is a FREE service for people living in the municipalities of Darebin and Whittlesea who have Type 2 diabetes. The service provides an integrated and multidisciplinary approach to patient management and links GPs to coordinated supporting health services. nDiabetes Direct accepts referrals to provide an integrated multidisciplinary and holistic approach to diabetes management.nOther services provided include endocrinology, dietetics, podiatry, diabetes educator, opthalmology, physiotherapy and ATSI liaison.
