About Disability Case Management and Brokerage Programme

This service provides case management and brokerage support to people with a disability. The program will be offered for a period of 6 to 12 months. Referrals are received and reviewed regularly by priority of need where vacancies exist. Once referral is accepted, a case manager is appointed. An assessment is done and an individualised case plan is developed. Case managers will work with the client and their family to co-ordinate a case plan and link to existing services, as well as purchase services through brokerage. The individualised case plan is reviewed regularly to address changing needs. Referrals can be made by phone, fax and/or mail and can be made by relevant stakeholder (eg individual, family member, carer, schools, disability services, juvenile justice, police).
